For the Residents of Settlers Campus (Settlers Landing and Settlers Place) Service Description

Settlers Place, as part of Settlers Campus, is a traditional condominium complex that will allow for independent living in separate condominium units within a larger supported community atmosphere. As is typical for this type of complex, it will be governed by a condominium association that will make decisions as to who will perform property management. Occupants of units at Settlers Place will purchase individualized traditional Engage services and Settlers Place Membership fee.

The following is a description of the services that are required by all those living at Settlers Place and Settlers Landing, whether owners or renters. The following services are necessary and required as part of the purchase agreement or as part of any lease agreement.

I. Traditional Vista Engage Program Services
Each occupant of Settlers Place and Settlers Landing will receive services from Vista such as employment placement and support, life skills support and program counseling. The level and type of service will be dependent upon the needs of the individual and the agreement made by representatives of Vista, the individual and the family.

II. Community Living & Social Support
All occupants of Settlers Place and Settlers Landing will pay a Membership fee to receive community living & social support services. The monthly Membership fee paid to Vista Life Innovations provides for the following:

Weekly on-site (community) group programming offered by Vista staff. Note: This is distinct from and in addition to any individual services such as life skills instruction that a resident receives. This on-site programming may be adjusted based on the participation and feedback of the residents.

On-call Resident Assistant based on the following principles.

Vista Engage Program Philosophy
All members of Vista's Engage program have demonstrated that they can live in the community without 24-hour support/supervision. Vista does not provide 24-hour supervision in any of its programs and most notably in the Engage Program. All members of the Engage Program do receive 24 hour On-Call Supports. They access this by calling Vista's main number (860) 399-8080 and speaking with a staff member. The staff member will determine action, if any. For all Engage members living in the community, there are Overnight Response services available. A staff member may go to the locale of the Vista Engage member and assist the member as needed. This will not happen automatically with every phone call but is an option at the staff member's discretion as he/she provides support to the member who is calling in.

Role of the on-site Resident Assistant at Settlers Campus
At Settlers Campus, there is an overnight Resident Assistant to provide emergency support and guidance as needed between the hours of 11pm and 7am. This Vista staff person resides in a Vista-owned condominium at Settlers Campus. The cost of this service is included in the Settlers Campus monthly Membership Fee. The Resident Assistant provides on-site support at the direction of the Vista Overnight Team Leader or directly in the case of an emergency.

As is standard procedure, all residents of Settlers Campus utilize Vista's standard protocol for accessing on-call supports during the overnight hours (11pm to 7am). If there is a concern, Vista Engage members should contact Vista staff by calling Vista at (860) 399-8080. The staff member will then determine next steps based on the situation. Next steps may include contacting the Overnight Response Counselor to intervene, contacting the on-site Resident Assistant and/or calling 911. In general, the Resident Assistant lives on site and would be available when directed to check on a resident by the Vista staff member.

When the Resident Assistant is planning to take vacation or be away, he/she makes Vista Leadership aware that they will be away and unavailable for a specified period. In the case of an urgent need at Settlers Campus, the Vista staff member will seek the services of the Overnight Response Counselor, following the existing protocols in place for all Engage Program households.

At the beginning of each month the Resident Assistant will publish their scheduled time away for the month and distribute it to Vista Program Counselors, residents of Settlers Campus, and post it in the common area of the building. This schedule will include information on how to contact the Vista Overnight Team Leader and/or 911 services if warranted.

Vista Program Counselors will periodically work with the residents of Settlers Campus to remind them and instruct them as to how to handle emergency or concerning situations as needed.
